Due to customer demand, Accuhealth is pleased to announce today its concierge-level Chronic Care Management (CCM) program, a fully managed service, which will be key in continuing to improve patient outcomes and reduce patient care load for providers.


After years of research and identifying gaps in the CCM delivery model, as well as hearing feedback from customers about issues with CCM, Accuhealth has built a program that easily ties into its software and takes all of the auditing, thinking, and billing out of an already complex program and makes it easy for physicians to utilize and gain data about their patients.

The CCM program generally provides non-face-to-face services to Medicare beneficiaries who have multiple (two or more) chronic conditions expected to last at least 12 months. Patients in their hospital-at-home can expect concierge care coordination, daily health and preventative medicine assessments, a 24/7/365 clinical care team available to them, intimate health coaching and motivation, and telemedicine and remote monitoring, including psychological monitoring. Setting SMART goals and appointments, care plan communication, patient education, and annual wellness visits are also included.

The award-winning platform automatically tracks all activity and interactive time, and produces a monthly CCM patient activity report and a monthly billing report to ensure each clinic’s compliance and monthly reimbursement.
