Accuhealth Remote Monitoring & Care Management service utilizes our Evelyn technology to keep patients healthy and in the continuum of care of their provider, reducing both direct and indirect costs incurred by costly and avoidable ED visits and hospitalizations. Our fully integrated model includes medical monitoring devices,  supplies, health signals, analytics, clinical intervention, EHR integration real-time patient education and coaching.

Tier 1- Observational Level Monitoring services include real-time monitoring with alerts and reporting to the Payor and/or Provider

  • Provider and Care Team portal access to Evelyn

  • Escalation and outreach to the provider and/or payer ONLY when the employee conveys symptoms to reduce avoidable and costly insurable events by Phone/SMS/Email (Clinical Escalation runbook is customizable at the respective Provider/Payer Level)
  •  Personalized Krames educational material for managing chronic diseases.
  • Accuhealth staff is bilingual (English and Spanish) and can support any language preference of the patient and/or caregiver

Initiation For new patients or patients not seen within 1 year prior to the commencement of RPM/CCM, Medicare requires initiation of services during a face-to-face visit with the billing practitioner. Has to be initiated by an Annual Wellness Visit, Initial Preventive Physical Exam, etc.
